When the perfect start isn't enough: tracing a toddler's eczema back to the gut

By every measure, her daughter Maya was set up for a healthy start. And yet, from around nine months old, eczema took over Maya's arms and the family's daily life. By most measures, Maya's start was close to textbook: born vaginally, breastfed for a year, never given antibiotics, and eating the kind of diet – fibre-rich, no added sugar, no white carbohydrates, no ultra-processed food – that many parents aspire to and few achieve.
So when eczema appeared on her arms at around nine months old, it didn't fit. No other symptoms, no obvious explanation – just red, bumpy patches that itched relentlessly.
"She was always scratching and bleeding. I was constantly having to cover her arms up so she couldn't get to them. All I thought about were her arms, and slathering them in the latest 'miracle cream'."
There was one possible clue in the timing. The patches emerged around the same time Jess introduced a single daily bottle of goat's milk formula, something she'd done to gently reduce her own milk supply, not out of necessity. Maya later self-weaned from it, but the eczema stayed, and that detail became a quiet source of guilt.
"Because I had a feeling it was to do with the formula, I felt a huge amount of guilt – especially because we didn't 'need' it. The mum guilt and stress was so, so real."
The day in the car
One moment captures how overwhelming things became. Jess was driving home from a family lunch, just her and Maya, when she heard her daughter say, in her small third-person way, "Maya is scratching."
"I turned around to see blood everywhere. All over her clothes and hands. I nearly screamed with shock."
She pulled over and climbed into the back seat. Maya was frightened by what she'd done, and Jess had to hold it together – reassuring her daughter while privately reeling.
"I wanted to hide away and cry, but I had to take some deep breaths, put a cardigan on her to cover the arms up, and get us home."
A cabinet full of creams, and a fear of steroids
Like most parents facing persistent eczema, Jess tried everything. "You name a cream that's been recommended for eczema and I've tried it. I spent hundreds." The GP offered steroids and emollients, the standard pathway, but a family member had been through severe topical steroid withdrawal, and the fear of putting Maya through anything similar made Jess unwilling to continue. A four-week trial of cutting eggs from Maya's diet changed nothing.
What frustrated her most was that nothing felt holistic. She sensed the problem was internal, but Maya's diet was excellent and the family lived a deliberately low-toxin lifestyle, so she'd begun to wonder if they were simply unlucky.
"GPs and dermatologists have a set pathway they follow that involves only medication, so naturally you think you have to follow it. Not once did they mention the gut microbiome – but that was our missing link."

Bifidobacterium is central to gut barrier function and immune regulation in early childhood, exactly the kind of bacteria you'd expect to be involved when the skin's immune response keeps misfiring. For Jess, who had long suspected the cause was internal, finally seeing it on a test was the breakthrough.
"We would never have guessed Maya was low in some strains, because I feel like she's an anomaly given her history and diet. It's something I could never have predicted – and it was invaluable to see it on a test result."
The action plan that came with the results was a mix of the simple and the genuinely challenging. Introducing a specific probiotic to address the low Bifidobacterium felt immediately achievable. Improving an already excellent diet was harder.
On her coaching call, the nutrition coach agreed Maya's diet was already very good and difficult to optimise further. The practical takeaways were to add more fermented foods – and to vary the oily fish in Maya's meals, rotating in options like mackerel rather than relying on salmon alone.
"Sauerkraut isn't readily available where we live, sadly, so we've opted for kefir and try to get that into smoothies two or three times a week," Jess says. The oily fish switch is a work in progress: "That's harder for me, because I only like salmon, so I need to get my head around it."
One detail reassured her: the recommended probiotic turned out to be a brand she already used herself. "So I trust it too."
Why a 'perfect' start isn't a guarantee
Maya's results sound paradoxical, but they reflect something microbiome researchers have been documenting for years. Bifidobacterium was long assumed to be present in virtually every breastfed baby, yet recent large studies show that is no longer reliably true in industrialised countries. In the UK's Baby Biome Study, the milk-adapted Bifidobacterium most associated with a healthy infant gut turned out to be rare; in the United States, the My Baby Biome Study found a sizeable share of infants – roughly one in four – whose gut composition left them more vulnerable to allergies, eczema, and asthma. In both, most of those babies were breastfed.
The likely reason is generational rather than individual. These bacteria pass from mother to baby, and decades of antibiotic use and shorter breastfeeding have gradually thinned that inherited reservoir. A baby can do everything "right" and still inherit a gut missing a key species – simply because it was already missing a generation earlier.
That matters for skin. Bifidobacteria help train the developing immune system, and lower levels in early life have repeatedly been linked to a higher risk of allergic conditions, including eczema – with long-term follow-up studies tracking that raised risk into childhood. It's exactly the kind of connection Maya's results pointed to: a quiet gap with outsized consequences, invisible without a test.
Small changes, and what happened next
The change wasn't overnight, and Jess is careful not to overstate it. The eczema isn't completely gone. But it is, in her words, "so much better" – and for the first time, the family feels at ease, knowing they've found something that genuinely helps.
It improved gradually. The redness began to fade. Maya scratched less. And she stopped reaching for cream, which had always been her default when her skin felt uncomfortable.

The moment that made it undeniable came from outside the family.
"Our swimming teacher noticed a change and asked what we'd done. I said it was the probiotics – and I couldn't quite believe it myself."
Beyond Maya's arms, something shifted for Jess too. The test gave her back a sense of confidence as a parent.
"You start to doubt yourself so much when GPs tell you the only way is steroid creams. This journey showed us otherwise. It makes you lean into your intuition a bit more."

She's now planning a follow-up test to see whether the changes show up measurably in Maya's gut, and to fine-tune from there. Her advice to other parents in the position she was once in is unequivocal.
"Absolutely try it. You have nothing to lose, because the more you know about your child, the better. Even if their microbiome isn't the cause of their eczema, it's one thing to rule out."
Asked to sum up the experience in a sentence, she doesn't hesitate.
"It's been completely eye-opening, seeing how powerful healing the gut can be in relation to eczema."
